Initial Consultation

Includes Interview/Intake of Information, Initial Meeting, Telephone & Face-to-Face Follow-up.

Placement Consultation

On-site visits with POA/Decision Maker, provide comparative analysis, Telephone & Face-to-Face Follow-up.

Speech/Language/Cognitive Assessment

Includes appropriate formal test procedures, informal assessments including but not limited to check writing, money management, pill dosage, telephone use and full written report provided.

Speech/Language/Cognitive Therapy

Including external aides i.e. scrapbook, journal, pill administration plan, home modifications, etc.

On-Site Visits

At-home: Observe client with caregiver, observe at mealtime to screen for Dysphagia, and provide follow-up report to POA/Decision Maker and any additional family members.

Facility: Observe client with caregivers during 7-3 shift, observe mealtime, participate with client in Social Activity and provide follow-up report to POA/Decision Maker and any additional family members.
